Healthcare Surrogate

A Healthcare Surrogate names someone to act if you become unable to make medical decisions for yourself. Copies of your healthcare surrogate should be provided to your primary and specialist doctors. Our office maintains an original in our file so that if you are need of a replacement or you are out-of-the area and a copy is required our staff, upon proper authorization, can provide that document to doctors and medical providers.
